Making a Difference in Patient Lives: A Story of Care
Jazmine, an All Care Health Navigator, recently shared this story:
“When I first started at our health center, I was introduced to Rogene, a patient who faced many challenges and was especially hard to contact. Rogene had no phone and didn’t seem interested in obtaining one. But as I met with her and gained her trust, I realized her challenges stemmed largely from needing answers to unspoken questions. She also needed reassurance and reminders about her health recommendations and appointments.
I took my time with her, and we eventually got her approved for free phone coverage through the Life Line Network. I happened to have an old phone and donated it to her so she wouldn’t need to find a way to pay for one. Having a phone has made a massive difference in Rogene’s health journey. We’re able to stay in touch easily to discuss her medication, treatment plan, and appointments. Most importantly, Rogene knows that at All Care, she’s supported in every way possible.”
