Making a Difference in Patient Lives: Annie’s Story of Care

Every patient’s health journey at All Care Health Center is an opportunity to give meaningful care. Jazmine, a Health Navigator at All Care, recently shared an inspiring example of how her role, in partnership with our providers, helps patients overcome challenges and achieve stability in their care plan.

Ben, a medical provider at All Care, was eager to help Isabel* better manage her diabetes by using a continuous glucose monitor at home. Isabel’s family had attempted to navigate the process independently but faced obstacles. When Jazmine reached out to Isabel and her family to fully understand the situation, she quickly realized that language barriers may have been preventing them from successfully obtaining the device.

With the help of All Care’s MARTTI interpreter services, Jazmine was able to communicate effectively with Isabel’s family and gather the necessary information. Shortly thereafter, the order for the continuous glucose monitor was completed, and the device was shipped directly to Isabel’s home. Grateful for the support, Isabel shared how easy the monitor was to use. She expressed her appreciation for All Care’s assistance, knowing she could turn to us for any future concerns.
